Identify Slate Pool Table with Ball Return

I am trying to find any information I can about this pool table. It came with the house when I bought it, and the previous owner bought it used from someone else and had no information about the pool table.

I know it has 1" slate (not sure yet if it is a one-piece slate or a three-piece slate), wire ball return and is a standard 8' pool table.

I would like to replace the pocket liner and gully boots, but I can't seem to find anything comparable. The tops of the boots are cracked and broken and don't completely cover the sharp points of the corner metal trim (which is quite sharp and I have some scratches to prove it).

The legs are solid wood and the base appears to be two plywood sandwiches running the length of the table with particle board bracing (one of which is cracked and needs replacement.) There is also some particle board mounted to the bottom of the slate, but not around the pockets. The felt around the pockets looks like it was glued to the slate at one time.

I have seen a similar billiard table once from a google search, but the owner didn't respond to any of my inquiries.
