Identify a J&J Pool Cue Model

I am trying to identify a J&J pool cue as shown below.

I am interested in finding out what the J&J model number is of the cue, the year it was made, and also what this J&J pool cue is worth.

    This is a J&J D640 pool cue.

    It was a low-end J&J "decal cue" sold from between 2002 and 2005. Retail was $69 at the time, but they would have a value of maybe half that today, if in perfect shape (which yours is not).

    From the 2002 J&J Sales brochure:

    D640 - Decal Cue, maple wood, with Linen Wrap - $69
